I Hate My Glasses Video Contest to Win Free LASIK Vision Correction

One humorous LASIK video and one serious LASIK video will be chosen. Submitted You Tube videos must be at the minimum 2 minutes in length but no longer than 4 minutes.
We know every video requires some work on your part, so everyone who submits a video will be sent a coupon via e-mail for $500 off bilateral Custom All Laser LASIK surgery. ($250 off per eye – may not be combined with other discounts or offers, no cash value, etc.).

4 finalists will be chosen by Dishler Laser Institute. Finalists must agree to come into our office to determine their candidacy for LASIK. The 2 winners chosen must be a candidate for LASIK, and agree to the terms and conditions of the contest. The two winners will receive Free Custom All Laser LASIK performed by Jon G. Dishler, M.D. (surgery is non transferrable, no cash value).

Winners must agree to appear on Martino TV before,and after their surgery and agree to have their surgery day pre-taped to show at a later date.
By entering the Dishler Laser Institute “I Hate My Glasses Contest” entrant agrees to have their video posted on the www.dishler.com website and possible showing on Martino TV. The 2 runner-ups will receive LASIK at 50% off our regular price of LASIK.

Contestants must be 18 years or older, a Colorado resident, must meet the criteria for LASIK candidacy, and all submissions must be received by midnight February 28th 2010. Dishler Laser Institute is not responsible for lost video submissions, and employees and family members of Dishler Laser Institute, Fox 31 and CW 2 are not eligible. Top 4 finalists will be notified by e-mail by March 8, 2010.
